How do I convert my physical shares to Demat account with name mismatch?

In case of name mismatch in physical shares certificate and demat account, you have to follow some procedures to convert your shares.
For example, your name on Physical shares certificate is Srinivas and Name on your Demat account is Sreenivas.
Then an Affidavit for name mismatch is to be taken duly attested by the notary on a Rs.100 INR bond.
Attach the affidavit to the documents required for converting the physical shares to demat account and submit them to the broker.
